Registration Forms

All enrolled AUCP families must submit certain forms before the beginning of the school year to meet regulatory and administrative requirements. Families should have been sent paper copies of each of these forms in the enrollment packet, but if you are missing a form, or need an extra copy, you will find the forms below.

All forms are available in Adobe PDF format, and many of them are available in Microsoft Word format.
